Medication Management

Our psychiatric mental health practitioners are trained to attend to mental health issues across the lifespan. They are qualified to carry out examinations and mental health assessments, diagnose psychiatric illnesses, prescribe medication based on a treatment plan that is unique to your physical health, and coordinate services for other healthcare components, including psychotherapy.

Wilmington Mental Health’s practitioners provide health care services directly to individuals being treated for or suspected of having physical or mental illnesses. Our patient care is both preventative and therapeutic in nature, consisting of direct supervision of various mental health issues to ensure the right medication is being used and to help reduce or eliminate symptoms effectively. Still, our team is deeply aware that medication treatment varies from individual to individual and requires interdisciplinary teams to work together to increase the efficacy of medication treatment.

Psychiatric Assessment

The first step of treatment involves conducting a psychiatric assessment, or psychological screening, to gather information about our patients and identify possible diagnoses.

During the assessment, questions about past medical history and use of medication are asked, in addition to any history of mental illness or mental disorders in the family. Previous psychiatric assessments, treatments, and degree of adherence to past treatments are also reviewed, and records from such care are obtained from places that hold those records. The method of assessment depends on whether the presenting problem is reported during a routine visit or a crisis. During a routine visit, target questions may focus more on the general medical and psychiatric history and evaluate several domains of mental function, including speech, emotional expression, thinking and perception. During crisis or emergency visits, the assessments may focus more on immediate symptoms and behaviors.

Ongoing Monitoring

Our medication management program involves initially evaluating the needs for psychotropic medications, followed by the provision of a prescription, and ongoing medical monitoring of the use of those medications.
Our prescribers are conservative with the use of psychotropic medications and will conduct a thorough review of the possible side effects medication may have and which drugs may fit best the person’s needs. Our providers will closely monitor the effect medication is having to help our patients achieve their desired outcomes.

Who should consider medication treatment?

Every individual is unique and psychiatric medications react differently within their body’s brain chemistry. Our healthcare providers empower patients in understanding the risks medication can pose to their health, knowing what medications should be taken and what dosage may benefit an individual the most while thoroughly addressing issues and offering a medication management plan. Prescribers can offer details about your prescribed medications, including interactions with other medications, potential side effects, and other considerations that may affect recovery.

  • Understanding the potential risks from mixing medications with food or supplements is important not only for your safety, but also to increase the effectiveness of the medication you have been prescribed.
  • Understanding your role in managing your medications can be crucial for your recovery and overall mental health as it can help you quickly identify that you are not experiencing the outcomes you expected. On the other hand, you may experience health complications despite taking the right medications in the right doses.

Our providers can track your medication intake to ensure that you are doing so consistently and that any complication that may arise during your treatment is reduced. WMH aims to provide an effective medication therapy management plan for you and your family.

Even though most people would prefer not to take meds, research shows that people start going “off track” with their meds as soon as they start taking them. Many people stop taking their meds within a year after starting them!

Our prescribers are conservative with the use of psychotropic medications and will conduct a thorough review of several prescribed drugs that may fit you and their possible side effects to create a treatment plan that is safe and effective. The initial assessment allows for the identification of physical condition and helps in the creation of the treatment plan. Your provider will closely monitor the effect medication is having on your body to help you get the desired outcomes.
